Passports
If you should lose your or your child's passport, please follow the procedures noted under How to Apply. In addition, you must also bring a lost passport report, which you can get from:

Bring the Entry/Exit Bureau lost passport report, and photo identification (such as a copy of the lost passport, or driver's license) to American Citizen Services unit.
We recognize that identification papers are often lost with a passport; however, applicants are also requested to provide, to the extent possible, evidence of citizenship and identity (e.g., original or photocopy of driver's license, expired passport, birth certificate, sworn statement of a U.S. citizen affirming applicant's identity, etc.). The fee for a replacement passport is U.S. $100.00 for adults and $85.00 for children (or RMB equivalent), cash or credit card.
Please note: our office can assist only in issuing a replacement passport. Following the issuance of a new passport, the Chinese government requires that a new visa be obtained for the replacement passport. The Shanghai Entry-Exit Bureau (see address above) can issue new visas for replacement passports. Failure to obtain a replacement visa can result in a fine when attempting to exit China.
